如何提交申索通知书

《弗吉尼亚法典》第15.2-209条要求，任何寻求对市政府提出过失索赔的人，应在事件发生之日起六个月内提供导致该索赔的事件的通知。

本通知必须以书面形式提交给市检察官办公室。 书面通知应包括：

1. The claimant's name 和 address;
2. 事件发生的日期；
3. 事件发生的地点； 和
4. 简要描述发生了什么，以及为什么你认为伦敦金融城应该对此负责。

收到通知和行动

通知必须在所述事件发生之日起六个月内收到。 Upon receipt, the 市检察院's Office will forward the Notice of Claim to the Virginia Municipal League Insurance Programs (VMLIP). The claimant will receive a letter confirming receipt of the Notice 和, if timely, will acknowledge the forwarding of the Notice to VMLIP. It is VMLIP's responsibility to investigate 和 evaluate the claim 和 determine whether any payment will be voluntarily made.
